NOTIFICATION OF INTENT TO

APPLY FOR A CERTIFICATE OF NEED

This is to provide official notice to the Health Facilities

Commission and all interested parties, in accordance with

T.C.A. §68-11-1601 et seq., and the Rules of the Health Fa-

cilities Commission, that MGA Home Healthcare Tennes-

see, LLC d/b/a MGA Homecare, a/an Home Health Agency

owned by MGA Home Healthcare Tennessee Holdings,

LLC with an ownership type of Limited Liability Company

and to be managed by itself intends to file an application

for a Certificate of Need for to remove the age limitation

(currently limited to pediatric patients pursuant to TCA

68-11-1607 (s) (1)) from its current Home Health Agency

license (#0000000640), so that the agency can provide

in-home home health services in Bedford, Cheatham, Da-

vidson, Dickson, Fayette, Rutherford, Shelby, Sumner,

Tipton and Williamson Counties, which will continue to

be self-managed. Its parent office is located at 1669 Kirby

Parkway, Suite 100, Memphis (Shelby County), TN 38120

and its branch office is located at 3310 West End Avenue,

Suite 500, Nashville (Davidson County), TN 37203. The ad-

dress of the project will be 1669 Kirby Parkway, Suite 100,

Memphis, Tennessee, 38120. The estimated project cost

will be $1,402,000.

The published Letter of Intent must contain the follow-

ing statement pursuant to T.C.A. §68-11-1607 (c)(1). (A) Any

healthcare institution wishing to oppose a Certificate of

Need application must file a written notice with the Health

Facilities Commission no later than fifteen (15) days be-

fore the regularly scheduled Health Facilities Commission

meeting at which the application is originally scheduled;

and (B) Any other person wishing to oppose the applica-

tion may file a written objection with the Health Facilities

Commission at or prior to the consideration of the appli-

cation by the Commission, or may appear in person to ex-

press opposition. Written notice of opposition may be sent
